We are pleased to offer online patient registration as well as online patient intake forms.  The online registration link allows you to enter your insurance and personal information live on the website portal.  Please download and print your intake forms prior to your appointment.

Please bring your forms, as well as your insurance card (if you are submitting insurance) and your I.D. to your first appointment. We look forward to working with you!

What To Expect

Your initial visit is designed to determine if you have a condition that our doctors can help you with.  In order to determine if you have a condition we can help you with, you will receive:

1. Initial history and consultation

2. Detailed and comprehensive examination including:
- Postural evaluation
- Range of motion testing
- Orthopedic testing
- Neurological testing
- Additional special testing as needed

3. Radiographic evaluation

4. Spinal stress tests if necessary

It takes time for us to properly analyze your examination and x-rays, so we will schedule you for a report of findings following your exam to go over your treatment options.  This allows the doctor to give you the best treatment recommendations specific to your condition.

Insurance and Payment Information

Insurance: Our office is a provider for many insurance carriers including: Blue Cross of Idaho, Regence Blue Sheild, Idaho Physicians Network (Cigna, Aetna, Health partners, Pacific Source, Ameriben, etc.), Micron Dakotacare/Wise network, First Health/Coventry Health, Medicare, Medicare and many more.  Please call our office if you have questions about your specific carrier.

Care Credit: For your convenience we offer care credit healthcare financing as an option for payment in our office.  This offers a variety of financing options including 0% financing options.  Please contact our office with questions or click on the link below to fill out your application.
